I hereby acknowledge and agree to the terms and conditions set forth in this liability waiver on behalf of myself and my child/children, participating in the StandOUT Youth Mentoring Program INC. ("StandOUT") located at the recreational center known as the MI3 Center.


Release and Waiver: In consideration of being allowed to participate in the Program, I hereby release, discharge, and hold harmless StandOUT Youth Mentoring Program INC., its directors, officers, employees, volunteers, agents, and representatives (collectively referred to as "StandOUT"), as well as the MI3 Center, its directors, officers, employees, volunteers, agents, and representatives (collectively referred to as "MI3 Center"), from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness that may be sustained by myself or my child while participating in the Program.


Assumption of Risk: I understand and acknowledge that participation in the Program involves certain risks, including but not limited to physical injury, illness, or property damage. I voluntarily assume all such risks on behalf of myself and my child.


Indemnification: I agree to indemnify and hold harmless StandOUT and the MI3 Center from any and all claims, demands, actions, or causes of action, including legal fees and expenses, arising out of or related to any loss, damage, injury, or illness that may be sustained by myself or my child while participating in the Program.


Medical Treatment: In the event of any injury or illness suffered by myself or my child during the Program, I authorize StandOUT and the MI3 Center to obtain medical treatment, including but not limited to emergency medical care, hospitalization, or surgery, as deemed necessary by medical professionals. I understand that I will be responsible for any medical expenses incurred.


Photographic Release: I grant StandOUT and the MI3 Center the right to take photographs or videos of myself or my child during the Program and to use such materials for promotional purposes, including but not limited to website, social media, and printed materials.


Governing Law and Jurisdiction: This liability waiver shall be governed by and construed in accordance with the laws of the state of Texas, without regard to its conflict of laws principles. Any legal action or proceeding arising out of or related to this liability waiver shall be brought exclusively in the state or federal courts located within the jurisdiction of Harris County, Texas.
